SOCIAL WORKER | HOSPICE AIDE | NUTRITION CONSULTANT


SOCIAL WORKER

POSITION SUMMARY

Provides services and/or establishes standards addressing the social and/or emotional factors that impact the clients’ ability to achieve the goals of the Plan of Care. Services are provided under the direction of a physician and in conjunction with other health care team members in accordance with the established policies and practices of the Agency.

QUALIFICATIONS

• Masters Degree from a school of Social Work accredited by the Council on Social Work Education.
• Minimum of one (1) year's experience in a health care setting or equivalent experience. Home care experience preferred.
• CPR certification, as applicable.
• Demonstrates excellent written and verbal communication skills and strong interpersonal skills.
• Demonstrates knowledge of resources available in the community.
• Licensed driver with automobile insurance in accordance with state/Agency requirements. Reliable transportation and demonstrated good driving record.
• Spanish speaking preferred


ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY

1. Assesses the psychosocial status of clients to determine factors that may interfere with client’s ability to achieve goals.

a. Completes assessments in a timely manner and in accordance with Agency policy.
b. Develops a written plan to provide information and direction to other health team members.
c. Documents all findings, plans, interventions, and client progress in the clinical record.
d. Communicates plans and changes to the physician and/or nursing supervisor and other Agency staff through the care plan, written progress notes, and participation in care conferences.
e. Initiates referrals as appropriate

2. Prepares social histories to augment existing service, or as a guide in determining or changing level of service. This may include cultural factors, financial concerns and support system.

3. Demonstrates teamwork and effective communication to accomplish client, team, and Agency goals.

a. Participates in care conferences and other team or Agency meetings. Shares areas of expertise and initiates interdisciplinary referrals.
b. Educates team members and clients/families about community resources and how to access them appropriately.

4. Supervises the assessment, planning and implementation by the Social Service team.

5. Provides leadership to the interdisciplinary team and the Advisory Committee in identifying opportunities to improve services, and implement strategies to address unmet needs of clients.

6. Assists in the development, implementation, and evaluation of the ongoing quality improvement process.

a. Serves on the Clinical Record Review Committee and Professional Advisory Committee; attends meetings as required.

7. Performs other related duties and responsibilities as deemed necessary.

HOSPICE AIDE

POSITION SUMMARY

Provides personal care services under the direction of the Registered Nurse or Therapist. The Home Health Aide is assigned to specific clients by the Registered Nurse or other appropriate professional and performs services for clients as necessary to maintain their personal comfort.

QUALIFICATIONS

• Successful completion of a formal certification training program and/or a written skills test and competency evaluation.
• Be at least eighteen (18) years of age.
• Minimum of six (6) months work experience in a supervised setting, preferably health care facility.
• Demonstrated ability to read, write, and follow a written Plan of Care.
• Good verbal, written, and interpersonal skills.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY

1. Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist.

a. Range of motion exercises
b. Assistance in ambulation or exercises

2. Performs personal care activities, including but not limited to:

a. Bathing
b. Shampooing
c. Skin care/nail care
d. Oral hygiene
e. Dressing

3. Performs household services essential to health care at home, including but not limited to:

a. Meal preparation/feeding
b. Laundry
c. Light housekeeping

4. Assists in the administration of medications that are ordinarily self-administered under the direction and supervision of the Registered Nurse (per state nurse practice laws and Agency policy).

5. Reports any observed or reported changes in the client's condition and/or needs to the Registered Nurse or Therapist. Follows reporting guidelines in the Care Plan.

6. Documents care provided and completes the forms required for the client's records. Completes the appropriate records to document care given and pertinent observations. Submits documentation per Agency requirements—at least weekly.

7. Promotes personal safety and a safe environment for clients by observing infection control practices, following Agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.

8. Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.

9. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.

10. Maintains confidentiality in all aspects of the job.

11. Attends in-service programs to meet compliance requirements. Identifies learning needs to Agency supervisor.

NUTRITION CONSULTANT

POSITION SUMMARY

Provides consultation and educational services to clients and Agency staff in the areas of nutrition, special programs, and meal planning.

Provides nutritional assessments for clients under a medically approved Plan of Care.

QUALIFICATIONS

• Registered Dietitian.
• Minimum of one (1) year experience in nutrition or dietetics. Hospital therapeutics, teaching, and/or public health preferred.
• Good verbal, written, and interpersonal communication skills.
• Current knowledge in dietary management of elderly, terminally ill, and therapeutic diets.
• Knowledgeable of community resources for nutritional guidance and support.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY

1. Assesses the nutritional needs of referred clients. Provides recommendations and education to clients and/or staff in accordance with the Plan of Care and the client's preferences.

a. Provides assessments within established Agency guidelines.
b. Recommends dietary modifications appropriate to the client's medical plan and the client’s cultural and personal preferences.
c. Teaches and clarifies complex dietary regimes for clients and families.
d. Documents findings, interventions, and plans in the clinical record.
e. Informs clients of community resources as appropriate.

2. Provides educational programs and resources for Agency staff on current nutritional practices and nutritional dietary management.

a. Reviews and evaluates nutritional education resources available for staff use.
b. Presents educational programs in identified areas of nutrition/dietary management.
c. Recommends educational and community resources for staff.
d. Responds to staff requests for consultation services related to clinical management issues.

3. Participates in interdisciplinary activities per Agency guidelines.

a. Attends client care conferences as requested to provide information to health care team.
b. Participates in Agency quality improvement activities as appropriate.
c. Participates in Agency meetings as requested.

4. Performs job in compliance with Agency policies and procedures, and professional and community standards.

a. Accepts responsibility in accordance with the role of Nutrition Consultant.
b. Provides services in accordance with applicable state, federal, CHAP, and Joint Commission standards.
c. Accepts responsibility for personal and professional development and identifies learning/developmental needs.

5. Maintains confidentiality in all aspects of the job.

Performs other related duties and responsibilities as deemed necessary.
